Hill grew up in Maine, USA. His first book, the short fiction collection 20th Century Ghosts (2005) won both the Bram Stoker Award for Best Fiction Collection and the British Fantasy Award for Best Collection, and his debut novel Heart-Shaped Box followed in 2007.

After the huge commercial and critical success of his first books, and following revelations about his family background in an article in Variety, Hill confirmed his identity as Joseph Hillström King, the son of one of the world's most famous living novelists Stephen King. His choice to use a pen name grew out of a desire to succeed based solely on his own merits.

Hill has published four novels, four short story collections and his fiction has appeared in various anthologies. His long-running comic book series, Locke & Key featuring the striking art of Gabriel Rodriguez, earned him the Eisner Award for Best Writer in 2011. It has been adapted to a major Netflix drama which aired in February 2020.
